It is important to note that the Coalition is association-based and not individual-based and as such, has membership form designed for intending association as shown below.

REQUIREMENTS
- Photocopy of Certificate of Incorporation/Registration.
- Two Passport size photographs of the President and Secretary.
- Tax Clearance (If any)
- Article and Memorandum of Association (Photocopy)
- The Total fees which is payable on submission of the Application is made up as follows:
